Required Documents for Medical Record Copies and Certificates



1. Relevant Regulations

Issuance is available only when the required documents for each applicant are provided, in accordance with Article 19 of the Medical Service Act, Article 21(1) regarding access to records, Article 13-2 of the Enforcement Rule, and applicable personal information protection regulations.

Applicant Required Documents
Patient Personal identification card such as a resident registration card or driver's license
Family Member
  • Patient's identification document
  • Applicant's identification document
  • Document verifying family relationship, such as a family relation certificate or resident registration record
  • Consent form for issuance of copies signed by the patient by hand
Authorized Representative
  • Copy of the patient's photo identification
  • Identification document of the applicant or representative
  • Consent form for issuance of copies completed and signed by the patient by hand
  • Authorization form for issuance of copies completed and signed by the patient by hand

2. Important Notes When Preparing Documents

  • Certificates include medical certificates, physician's opinion letters, and admission or discharge confirmations.
  • Identification must be government-issued and include the holder's photo and personal information.
  • Documents verifying the relationship to the patient must be issued in the name of the patient or the applicant and must clearly state the relationship.
  • Consent and authorization forms must bear the patient's handwritten signature. Issuance may be denied if seals, stamps, alterations, or corrections are used.
  • The consent and authorization forms follow the format designated by the Ministry of Health and Welfare. Printed forms may be prepared in advance, and copies are also available at the Administration Office.

Please note that issuance may be restricted if the required documents are incomplete.

1. Required Documents by Applicant Type

  • Documents confirming family relationship may include a resident registration record, family relation certificate, or national health insurance card
  • For minor patients under age 14, the consent form is not required

  • For patients under age 17 without a resident registration card, patient identification may be omitted
  • For minor patients under age 14, the forms must be completed by the patient's legal representative, and documentation confirming the family relationship must be attached

2. Required Documents When Patient Consent Cannot Be Obtained

Situation Required Documents
If the patient is deceased
  • Copy of the identification of the person requesting record access or issuance of copies
  • Document verifying family relationship, such as a family relation certificate or resident registration record
  • Document confirming death, such as a family relation certificate, removed family register, or death certificate
If the patient is unconscious,
or unable to provide a handwritten signature due to serious illness or injury
  • Copy of the identification of the person requesting record access or issuance of copies
  • Document verifying family relationship, such as a family relation certificate or resident registration record
  • Medical certificate confirming that the patient is unconscious or unable to sign by hand due to serious illness or injury
If the patient is missing
  • Copy of the identification of the person requesting record access or issuance of copies
  • Document verifying family relationship, such as a family relation certificate or resident registration record
  • Document confirming the missing status, such as a resident registration record or a court decision declaring disappearance
If the patient lacks decision-making capacity
  • Copy of the identification of the person requesting record access or issuance of copies
  • Document verifying family relationship, such as a family relation certificate or resident registration record
  • Copy of the court decision on legal incompetence or a medical certificate from a psychiatry specialist confirming lack of decision-making capacity
