Required Documents for Medical Record Copies and Certificates
1. Relevant Regulations
Issuance is available only when the required documents for each applicant are provided, in accordance with Article 19 of the Medical Service Act, Article 21(1) regarding access to records, Article 13-2 of the Enforcement Rule, and applicable personal information protection regulations.

2. Important Notes When Preparing Documents
- Certificates include medical certificates, physician's opinion letters, and admission or discharge confirmations.
- Identification must be government-issued and include the holder's photo and personal information.
- Documents verifying the relationship to the patient must be issued in the name of the patient or the applicant and must clearly state the relationship.
- Consent and authorization forms must bear the patient's handwritten signature. Issuance may be denied if seals, stamps, alterations, or corrections are used.
- The consent and authorization forms follow the format designated by the Ministry of Health and Welfare. Printed forms may be prepared in advance, and copies are also available at the Administration Office.
Please note that issuance may be restricted if the required documents are incomplete.
